In the form, it states "Personality/History". That means you can put either Personality OR History, not both unless the person applying wants to put both.

Yes, but seeing as Calypso and some others wrote NIETHER, their form was rejected. Sorry if that sounds a bit harsh, but it's just sometimes the way I do things.
