I never noticed the doctor either but all my Sherlock fan friends agree with the ball theory (and a science teacher confirmed to us that it is possible). Also, if you look closely, when he lands there is no blood around him at all but, when they cut back after Watson has been knocked over, there is lots of blood. That amount of blood couldn't have formed in that short space of time so our theory is that the crowd poured it there. One of my friends also had an interesting theory; you never see the impact so she thinks that he actually jumped into the rubbish lorry (which was between Watson and the 'point of impact') which would have cushioned his landing and then jumped out onto the floor to appear dead.

This is completely off topic but this earlier post says that it has been edited 999,999,999 times (i.e. nearly 100 million times)!!! I think there may have been a computer glitch there!!

Yeah, I noticed that with the blood as well. I'm not sure whether it's a glitch or not, but when Watson gets hit by the bike, the van is gone and there's loads of people around Sherlock.
But then there's an ariel view, and the van's still there, and only a few people around him.
Could be a filming error, but I'm not sure.

Really? Did you read that in an article? If so, can you provide the link.

It wasn't in an article - Steven Moffat and Mark Gatiss said that it was commissioned at the same time as series 2 on twitter.

I've just recently watched the TV series of Sherlock and really love it c:
I don't really have a detailed theory of how he survived the jump from that building but I think it has something to do with the H.O.U.N.D drug, I think it was a drug, sorry if I'm wrong xD
Also I haven't read the books so I don't know if this was explained in them, but did Sherlock really pay I think it was Richard Brooks to be Moriarty? Or were they just trying to fool him?
Anyways, can't wait for season 3! C:

Now, i have a few ideas how he faked the moment when John took his pulse. Of course, they have been put in my head by the holy land of Tumblr.
1. The ball-in-the-armpit
2. The Rhododendron ponticum, a plant that Sherlock actually mentioned in the episode earlier. As i've understood, it makes your pulse slow down drastically (i might be wrong, but that's how i understood it) and causes runny nose and watery eyes.

I have no idea how he survived the fall itself, though i'm 90% sure that Molly and the homeless network have something to do with it.

And i'm quite sure that every time Moffat and Gatiss see that we're getting close to how Sherlock faked it, they just rewrite the 3rd season.

I have to say, I'm insanely jealous of my friend. She was in the queue in a shop whose name she won't share, and was actually standing behind the actor who plays Moriarty. Very jealous.

I honestly don't know. I do not think its the H.O.U.N.D drug because it just caused side-effects and uncontrollable violence.
