Sherlock asked Molly to supply a dead body/ dummy for when he was "dead" on the ground. He told Watson to keep looking at him, right? So he couldn't see Molly leaving the body/ dummy. And the connection of homeless people could easily have been paid by either him or Mycroft to keep John from seeing him properly. Because Molly wasn't a target for the snipers, it meant she wasn't being watched, so easily could have helped him. The rubbish truck was parked in the way, and drove off as the crowd began to gather. Still, he could have used the H.O.U.N.D drug to trick Watson's mind into thinking it was Sherlock. With the girl, who screamed when she saw Sherlock. Moriarty must have done something to make her act like that, right? Well, what if Sherlock used the same techniques in his "death"?
Mycroft also didn't seem bothered when reading about his brother's death, and acted as if he was just reading the weather forecast in the newspaper.
Another thing I was thinking was that when Watson rushed over, he checked Sherlock's pulse and found nothing. Well, do you remember that ball Sherlock was bouncing beforehand? He could have put it under his armpit to stop his pulse being able to be read. (Trust me, this works...)

How could Sherlock pretend to be dead with blood covered all over his face if he was unharmed? Surely he couldn't of gone so quickly to the place where he "fell" and put blood all over his face in such a realistic way. The person laying on the floor mustn't of been Sherlock himself, so perhaps it was a body or a dummy with fake (or even real) blood dressed to look like Sherlock?
Also, about the girl screaming when he saw Sherlock (even though she'd never seen him before), could Moriarty of had a mask that looked like Sherlock's face on? Perhaps Molly (or Sherlock) put the mask on Moriarty's body and threw it down?
I'm not sure. But whatever happened, it doesn't really matter, 'cause he's alive! Harr.
