Well...
Finnick could only carry Peeta and Katniss could no longer carry Madge and Madge was old and did not want the "Kids" to die from the fog so she knew she was old so walked into the fog to spare them from having to carry her longer...
I think that's why she died...

Madge died during the bombing since they couldn't find the Mayor/his wife I'm assuming they did not find Madge. So yes , she did die.
