The ribbons in her hair to me look like people ((I know i am weird)) So I think that the girl has seen the demon thing approaching her and is talking to the two aspects of her mind, the good side and the bad side.

I really like this drawing though, that's just my odd interpretation of it.

Or maybe it could be Masaomi Kida, who has decided to dress up as an angel to get girls, and he's trying to protect Harima, who had gotten out of her weird boyfriend's sights for just a moment to get a drink, and somehow ended up in the middle of a fight between Izaya and Shizuo. (shizuo would be the one with the demonic aura back there, yo)
The conversation would go something like this:

"Heeeey, you're pretty cute."
Harima screams.
"Whoa, why are you screaming?"
Harima points to the demonic-looking, blonde-haired man coming towards them at an alarmingly fast rate with a vending machine in one hand, and then to a dark-haired one that's smiling.
"IZAAAYZA!"
"Oh crap," Kida says.
