First a few rules:
1) You may pretty up your form, but if I can't read it (small pale font is a no-no) I can't judge it.
2) You may have one extra (one piece of art, one poem, etc. Get creative!). This is optional but a definite boost if you decide to do it.
3) Depending on the number of entrants, any WIPs after the end date may be disqualified.
