hi, i'd like a character form.

character's name; i don't really have a specific character in mind yet, i was hoping you could make a somewhat complex form that i can use for multiple different characters if i wanted lol. if that can be done? i can code my simple forms with basic boxes but i don't even know where to begin on a more complex one.
about my character; text text text
color scheme; red, black, gold
image/gif of my character; was kinda hoping for a image space about 200 x 200 or 430 x 180.
anything special i want done with character form; i would like room for the basics, name, sex, age, breed, rank, and maybe some space for strength // speed // stamina // pain tolerance // fighting // medicine etc. and some space for personality/history, possibly likes and dislikes.
payment; this guy, this little one, and this one
anything else way should know; i hope this is okay? i'm not really sure what i should specify for. if i need to elaborate on something i would be happy to. i just want a more vague, complex form i can use for multiple chars.
