Here's a form a did for another kind, but I did edit it a bit. If the signature isn't 'what I expected'(not to be rude) I will still give you a rare (unless it's TERRIBLE) but it might not be a 2010 one.

About medium should be fine.

I'll only be putting around three links, so small, please.
