I got the UR grapes for the rerelease, and I'm wondering if is it common for the UR Grapes be swapped for a Sorbet with nothing else added? I know fruits tend to have bad demand and the sorbets have pretty good demand... how much would have to be added to make the trade fair?

generally speaking, it is really really hard to just do a straight swap with fruits, but most of the time if you just keep at it, and just keep trying for a tier swap it will happen eventually. Part of the reason why fruit are hard to trade is just that it takes a long time and people end up adding more to go with their fruit just to trade off quickly, which then has a flow on effect and makes it expected for other members to need to add more.

If you really just want a tier swap you most likely will eventually, its just dedication.

I haven't seen this pet anywhere on the demands list, is it worth a lot?

I'm not sure what you're talking about as in the demands list but I assume you mean rares list - it's the UR Rat, at the blue toxic tier, so yes it is worth a lot. However you might find it hard to do a direct swap or trade it away as a lot of people seem to value rats less than other species.

It says it's an OMGsr, I'm confused XD

The OMGSR tag is given when there are less per amount of users. Since the rares list takes into account both demand and actual rarity, some OMGSRs might be at lower tiers of the main list instead. Low demand pets might be OMGSRs and still be worth less than a very rare list pet.
